Welcome to Normal Park Lightning Athletics

Welcome Parents and Athletes!

The following forms are required for your student to participate in athletics at Normal Park. The forms must be submitted and cleared prior to the try-out.

  1. Medical History – to be completed by the family prior to physical appointment
  2. Physical Form – to be completed by a medical provider
  3. Medical Eligibility – to be completed by a medical provider
  4. Cardiac Arrest Statement – to be completed by the family
  5. Concussion Statement – to be completed by the family
  6. Consent to Participate – to be completed by the family

The district is no longer partnered with Dragonfly. While we wait for the new system to be up and running, please complete all six forms and submit them to the coach prior to the sport try out period. Physicals must be completed after April 15, 2023 for the 23-24 school year.

Sports Fees

The Sport fee will include facilities fee, insurance, refs/umpire fees, uniforms. A Lightning Season Pass will also be given to the immediate family of all student athletes who pay sports fees prior to the first home game. This pass will provide immediate families with free admission to HOME ball games during that specific sport. (tournaments excluded).Shooter shirts, extra equipment, camps, and warm up's may result in an extra fees.
