Important Forms

All students trying out for a sport must have a physical before tryouts. If in grades 2nd-5th your forms must be turned into Tiffany Phillips at the lower school. In additional to a physical parents must complete the Important Forms below.

TSSAA Physical Form and Medical Eligibility forms are the only ones you'll need to have completed by your doctor during a physical exam
